Health One

In response to an identified need for an integrated primary care centre in the Canterbury region, GP Synergy, in consultation and collaboration with key stakeholders, has embarked on the development of a HealthOne primary health care facility in the Canterbury LGA. HealthOne Canterbury is one of 13 HealthOne NSW services still under development around the state. HealthOne NSW is an integrated primary and community health initiative, bringing together GPs, community health and other health professionals in multidisciplinary teams. The NSW Government has committed $52 million in capital and recurrent funds over five years to 2010/11 to establish HealthOne NSW services across the state.

The model of care proposed is the co-location of Sydney South West Area Health (SSWAHS) - Community Health and General Practice at a single location (Proposed site - 63 Tudor Street Belmore, NSW). 

The proposed service model will integrate local public and private primary care services - GPs, community health, nursing, allied health and drug health services and establish a new integrated primary care facility which embodies and models best-practice, particularly in the area of chronic disease management.

HealthOne Canterbury has an endorsed model of care that was developed around the specific needs of population and disease groups. The project is in the final stages of capital works planning. NSW Health and SSWAHS recently affirmed  strong support for the realisation of the project with ‘in principle’ agreement to fund the capital works development pending revised architectural plans and business modeling to ensure viability and sustainability.
